Transition in Hospice Care Houston Texas
Accredited Healthcare of America is here to provide patients and their families with support and guidance in the transition from a hospital setting to a hospice program. We make sure they get the services, supplies and information they need to be comfortable, calm and confident during the transition from hospital to home.
Hospice Care Houston Texas – Hospital Consultation
We come to the hospital to meet you and your family which allows us to get to know you and let you know about hospice in general as well as the specifics of our program so you can determine if hospice care might be the next step for you.
Transitioning in Hospice Care – Move Coordination
Once you consent to hospice care, a hospice team member is always available from the beginning of the transition, to explain what is happening, what to expect and what to do.
Transitioning in Hospice – Medical Equipment
Transitioning to hospice care may include the use of special medical equipment. The hospice staff can help order, coordinate the delivery of and instruct the patient and family on the use of any medical equipment needed.
Hospice Care Transitioning – Medication Coordination
Medication coordination is also part of transition care. We can help organize medications, make sure refills are ordered and received and that medications no longer needed are properly disposed of.
Home Visits
Once the patient is settled at home and has been formally admitted into our program, hospice team members will visit based on the individualized care plan developed during the initial home visit. These visits will include continuous pain and symptom management, personal care, patient care instruction and family support.
Transitions can be stressful, but making the transition to hospice means you’ll always have a team of specialists to guide and support you every step of the way.
